Medicare Pays for
Older adults with multiple chronic conditions are eligible for additional Medicare paid services, with only a small copay. Our coaches develop a comprehensive care plan for these seniors, guide, motivate and simplify taking medications to enable them to live independently and avoid hospital stays.
